What role do comprehensive histories and physical examinations play in your practice’s standards of care?
 
According to David Robbins, DVM in the October 1, 2008 issue of Veterinary Medicine: “The physical exam is among the few commonalities in veterinary medicine, practiced by every veterinarian trained in every country. Anyone who has practiced or observed veterinary medicine in an economically challenged setting knows the physical exam is often the only method available for diagnosing a patient's illness.”
 
In this thought-provoking article, Dr. Robbins describes what he sees as a disturbing trend to devalue history-taking and physical examinations in some veterinary practices.
 
“It is the fusion of the physical exam, the patient's history, and technology that will help veterinarians become more accurate, quicker diagnosticians and healers while maintaining the crucial veterinarian-patient-client bond.”

Veterinary practices are currently operating in a volatile business climate. This article from the October 2nd edition of The New York Times aptly describes the present financial situation for small businesses - such as veterinary practices - in the US.

“Small businesses in America (27 million companies employing fewer than 500 people and in most cases – fewer than 20 people – account for half of the nations’ output. They employ 40% of the workforce and outpace large companies in generating jobs”.

Even if the financial “fall-out” from the current US economic crisis has not yet hit your part of the world – chances are it will affect your practice to some degree.
 
For US practices, the “credit crunch” can mean:
  • Higher rates and tighter credit will affect veterinary practices – increasing our costs and affecting growth.
  • As practices buy only what we need (“just-in-time” approach) – suppliers will reduce inventory and manufacturers will shrink production.
  • Our clients will be affected by the financial climate and many will reduce their discretionary spending.
 The bottom line for veterinary practices:
 
Using effective practice management and fee strategies iwill be critical to enable them to:
  1. Continue to offer high quality patient care and client service
  2. Attract and retain competent associates and staff and maintain a positive work environment
  3. Remain profitable in order to build value in the practice so that 1 & 2 can be accomplished!

Assume we could find a group of doctors, technicians, receptionists, and assistants who were willing to volunteer their time to work in our facility. In addition, assume our facility didn't contain any pharmaceuticals, vaccines, or equipment. We'd still have to ask our clients to pay us $900 a day for the smiles and advice we give them. And smiles and advice would be all we could offer.
